The normal human foot features an arch that helps provide stability and support to the body while also acting as a sort of shock absorber for walking, running and jumping. It results from loss of the medial longitudinal arch and can be either rigid or flexible.

The most common display of faulty foot biomechanics is ‘over-pronation'. Only 5% of the population is flat-footed, i.e. no arch is present whatsoever under the foot. Over-pronation causes the legs to rotate internally, which leads to an irregular motion in the knee joint and it also forces our pelvis to tilt forward which puts tension on the muscles in the lower back. Orthotics is designed to correct faulty foot function reducing the amount of ankle role and arch flattening during the gait cycle. By correcting over-pronation orthotics re-align the feet and ankle bones to their neutral position, restoring our natural foot function. Surgery is rarely needed for pes planus.

It was shown that the odds of having undergone pitching-arm surgery in pitchers with either type of abnormal foot posture (pes planus or pes cavus) in the stance foot were 3.4 times higher than in pitchers with typical foot posture. The odds ratio for pes planus on the stance foot was 3.7 and statistically significant, whereas for pes cavus the odds ratio was 3.2 and approaching significance.19 A pes planus foot lacks the stability of the inert tissues to maintain a proper arch. This may be especially true during the wind-up phase, in which muscle loading occurs from the ground up. Additionally, the wind-up phase prepares the pitcher for correct body posture and balance. Pitchers tend to dislike the feeling of an ankle brace due to a perception of stiffness and the constraint they feel during their delivery.

Other factors, such as the severity of family history of the problem, or severity of causative factors such as a tight calf muscle may also play a role in deciding when surgery should be considered. Surgery is rarely recommended for children under age 5, because it may take that long in some cases for an arch to develop. There are a variety of different surgical procedures that may be utilized when surgically correcting a pes plano valgus foot. The risks of pes plano valgus surgery include, but are not limited to: infection, nerve injury or entrapment, prolonged healing/recovery, wound or scar problems, incomplete relief of pain, no relief of pain, worsened pain, recurrent pain, calf atrophy, weakness, limping, incomplete arch restoration, failure of bone cuts or fusions to heal, intolerance of implants, stiffness, and transfer of pain to other sites or joints. Orthotics are typically worn with closed shoes. The posterior tibial tendon is an important support for the foot arch.

Patients with pain limited to the rearfoot were more likely to demonstrate an antalgic gait pattern, lower plantar pressures, and higher pressure-time integrals than those with pain limited to the forefoot. The same research group also published the first randomized controlled trial20 to investigate the effectiveness of custom orthoses for the treatment of painful pes cavus deformity. One hundred fifty four patients with chronic idiopathic foot pain and bilateral cavus feet received either custom-molded orthoses made of 3-mm polypropylene with a Poron top cover or sham insoles of 3-mm foam. Complete two sets of 10 repetitions on each foot.

However, when you have a flat foot, your mechanics in walking and foot alignment are usually poor. These orthotic insoles will have arch support to help you distribute your weight more evenly throughout your flat foot, or may accommodate the position it's in if you have a rigid flat foot. In rare cases surgery may be suggested when no other more conservative approaches have been successful, but usually not before a trial of foot orthotics / arch supports and supportive footwear. A flexible flat foot usually required a semi-rigid orthotic insole to correct the deformity. Overuse can also result in a significant amount of irritation and pain.
